Does a referral guarantee an appointment?
No. The practice must review the referral, service requested, availability and administrative requirements before an appointment can be confirmed.
Can we request a home visit?
Yes. Home-visit consideration may be requested, but availability depends on geography, clinical appropriateness, physician capacity and safety. Home visits are not emergency services.
Can a geriatric care manager or discharge planner submit?
Yes, when the person is authorized to coordinate care and transmit the information. The form distinguishes an authorized care-coordination request from a clinical referral made by or on behalf of a treating clinician.
How should records be sent?
Use the secure referral upload or another verified secure channel. Do not send patient records through ordinary email.
Will the referring office receive follow-up?
Relevant follow-up information may be communicated through secure channels when appropriate and permitted by applicable law and the patient’s preferences.
Does the patient need a membership?
No membership is required to submit a referral or request medically appropriate neurological care. Optional membership services are separate from medical visits and clinical decision-making.
