Referral Essentials, All in One Place.

Fax Referrals & Records

Fax: (833) 455-6495

Send through your organization’s approved fax process. Include a cover sheet and your office’s return contact information. Call us if receipt needs confirmation.

Request Coordination

We Can Contact Your Office.

Use the office callback form to request contact from our team. Do not enter patient names, medical details or records. Submitting a request does not confirm an appointment.

What should accompany a clinical referral?

Through the approved records route, include the referral question, relevant clinical notes and test reports, patient contact information needed for scheduling, and the referring office’s contact details. Send only records relevant to the request. Do not send medical records through ordinary email or this website form.

For your own appointment, please use our patient callback page. For a medical emergency, call 911 or seek emergency care.

Care That Complements Yours

A Thoughtful Partner for the Clinical Question in Front of You.

Primary Care, Concierge & Cardiology

Neurological evaluation and recommendations that support your patient’s ongoing care—from headaches and cognitive concerns to stroke prevention and risk review.

Home Health & Senior Living

Help coordinating neurological evaluation when mobility, cognition, transportation or caregiver needs affect access. Limited home visits may be considered.

Second Opinions & Complex Questions

Careful review of the history, examination and available records, with understandable conclusions and a plan for unresolved questions.

We also welcome inquiries from urgent-care teams, discharge planners and other authorized care coordinators. Care-coordination requests are distinct from clinical referrals.

Clinical Fit & Visit Options

Adult Neurology, With Clear Next Steps.

Not sure which service fits? Call our office to discuss the appropriate referral route. Clinical review and scheduling determine whether a visit can be offered.

Office, Video or Limited Home Visits

Visit setting depends on clinical appropriateness, patient location, safety and physician availability. Home visits are not guaranteed or emergency services.

Clear Payment Expectations

Our office explains applicable fees and payment arrangements before confirming a visit. Membership is optional and is not required to request medical care.

Insurance & Payment Information

Communication Is Part of Care

From First Contact to the Next Step.

Our goal is an administrative response within one staffed business day. This is not a promise of an appointment within that time.

Connect With Our Office

Call, request an office callback or send a referral by the approved fax route. Keep patient information out of the website form.

Coordinate the Handoff

Our team reviews the request and identifies the information needed for clinical review and scheduling.

Review Visit Options

If an appropriate visit is available, our team contacts the patient or authorized caregiver and explains the administrative next steps.

Keep Care Connected

Relevant consultation findings and recommendations may be shared through approved channels, when appropriate and permitted. Please identify your office’s return contact.

For referral status, missing information or a scheduling concern, call our office. A request does not transfer responsibility for urgent clinical needs.

Quick Answers

Before You Refer.

Does a referral guarantee an appointment?

No. The practice reviews clinical appropriateness, service availability and administrative requirements before an appointment can be confirmed. Call our office to discuss a time-sensitive concern; this form is not an emergency or clinical-triage channel.

How should our office send records?

Fax relevant referral records to (833) 455-6495 using your organization’s approved process, with a cover sheet and return contact information. Call (754) 216-2989 for questions or receipt confirmation. The website form does not accept medical records; do not use ordinary email for patient information.

Can a care manager or discharge planner contact you?

Yes. Authorized care coordinators may request help arranging access. A care-coordination request is not itself a clinical referral or medical order; our team can clarify the documentation needed.

Can we request a home or video visit?

Yes. These options require review of clinical appropriateness, location, safety and availability. Home visits are limited and are not emergency services.

How do we stay informed?

Provide your office’s contact details and approved return channel with the referral. Relevant consultation information may be shared when appropriate and permitted by applicable law and the patient’s preferences. Contact our team about referral status or unresolved scheduling questions.

Is membership required, and how is payment handled?

Membership is optional and is not required to request medically appropriate neurological care. Our office explains applicable fees and payment arrangements before confirming an appointment. Insurance coverage and reimbursement depend on the specific plan and service; see our insurance and payment information.
